Description Affinity Health Center is seeking a full time Community Health Workers. The Community Health Worker will seek to improve access to health care services for people living with diabetes or those at risk for diabetes. CHW will seek to bridge any gaps between community members and the health and social service system to enhance communication between people living with diabetes and those providing diabetes care. CHW will support patient and community access to education and care, helping to reduce health disparities in underserved communities.
Pay Range: $19.00 - $22.00 per hour based on experience.

Position Requirements - Experience related to diabetes or the care of individuals living with diabetes.
- Strong knowledge of and relationships within the local community to be served through this program.
- Must have a valid driver's license and reliable transportation
- Candidate will conduct home visits and community-based outreach
- Bilingual preferred
- CHW Certification preferred
