Job Description Summary:
JOB SUMMARY The Solid Organ Transplant Coordinator is responsible for coordination of the evaluation and preparation of patients for organ transplant and long-term management of patients post-transplant. The Transplant Coordinator is responsible for coordinating patient care across the continuum through inpatient and outpatient settings. The Transplant Coordinator should demonstrate independent thinking through a high degree of problem solving and innovation. These skills are required to resolve complex problems that arise unexpectedly and apply appropriate interventions to individual patients while performing daily tasks. The incumbent serves as a clinical resource for other team members, patients, and caregivers, referring providers, and community clinicians. Interacts daily with patients, families/caregivers, nurses, pharmacists, social workers, navigators, financial counselors, and physicians to provide and facilitate care. Provides direct patient care including education of patients and caregivers regarding transplantation including assessment, monitoring, and management of pre- and post-transplant side effects and complications. The Transplant Coordinator: · Interacts daily with patients, families/caregivers, nurses, pharmacists, social workers, discharge planners and physicians to provide and facilitate care. · Communicates and collaborates with community physicians, pharmacists, laboratory personnel, home health agencies, insurance company case managers and medical directors to ensure a successful continuum of care. · Demonstrates competence in managing the care of people with renal and hepatic failure, diabetes, cardiopulmonary disease, hypertension, hyperlipidemia, and other disorders related to transplantation. - Demonstrates competence in the management of immunosuppression, including a host of adverse reactions and drug interactions. This position requires organizational skills, problem solving abilities, and decision-making skills to support the planning and scheduling of transplant patients to ensure a successful continuum of care. Must be able to prioritize clinical issues and case manage patients in various stages of the transplant process including outpatient pre-transplant workup, inpatient, and long-term post-transplant follow-up. Responsible for some program development and research related activities.Responsibilities And Duties:
